HPR Authentication Required

To register a new healthcare facility in the Health Facility Registry (HFR), you must first verify and log in using your Healthcare Professional Registry (HPR) ID.

HPR ID / Practitioner ID * Use HPR number 71-XXXX-XXXX-XXXX or username name@hpr.abdm.

Select HPR Profile / HPID * Select the HPR ID profile you wish to log in with.

HFR to HPR / Facility Manager Linkage

HPR ID / Facility Manager ID * Pre-populated with active logged-in HPR session identifier.

Registered Mobile Number *

OTP will be sent to the registered mobile number.

Select HPR Profile / HPID * Select the HPR ID profile you wish to link to this facility.

Link Facility to Client Bridge (HIP / HIU)

New facilities registered from this portal are auto-linked to your Client ID via ABDM addUpdateServices. Use this form to manually link an existing HFR Facility ID as HIP and/or HIU under bridge SBXID_046771. Facility should be Approved in HFR (not only Submitted). If ABDM returns HTTP 503, the sandbox bridge-link API is down — retry later or link from the ABDM Sandbox portal.

Enter the HFR Facility ID (e.g. IN2710000059) to look up its current status and details.
The numeric Tracking ID is only used during registration and cannot be queried after submission.
